Proctored Exams at Grayson College Testing Centers - Agreement Form

Agreement

By requesting to take an exam at the Grayson College (GC) Testing Center, you acknowledge and agree to the following terms:

  1. Exam Materials: It is my sole responsibility to ensure that my college or university sends the necessary test materials and passwords directly to the GC Testing Center.

  2. Appointment: I will contact the GC Testing Center at least one week in advance to schedule an appointment for my proctored exams.

  3. Scheduling Availability: Proctored exams for other institutions will be scheduled on Monday-Friday during regular hours. I've checked the posted times and will finish my exam well in advance of closing. I am aware that the GC testing schedule may limit availability on certain days or weeks.

  4. Test Day Requirements: On the day of my exam, I am required to bring a valid photo identification and the $20 proctoring fee in exact cash. I understand this fee is one of the lowest in the state and is subject to change.

  5. Additional Fees: I understand that supplementary services provided by testing staff, such as administering multiple sections of an exam, or reading verbal instructions, will incur additional fees.

  6. Technical Disclaimers: I acknowledge that the testing center is not responsible for any technical difficulties that may arise with computer-administered tests.

  7. Center Policies: I agree to comply with all GC Testing Center rules and the Academic Integrity Policy.

  8. Cancellations: I agree to notify the Testing Center staff if I am unable to make my scheduled appointment.
